Mr. Formolo joined CHS in 1990 and was promoted to Partner in 1997. Prior to
joining CHS, Mr. Formolo was employed by Heller Financial,
Inc. as a Vice President in the Leveraged Funding Group. He
holds a B.B.A. from the University of Wisconsin and an M.B.A.
from Northwestern University. Mr. Formolo serves on the Board
of Directors of American Reprographics Company, AMF Bowling Worldwide, Inc., KBA Holdings, Inc. and Suture Express, Inc.
Mr. Formolo also serves on the Board of Trustees of North Shore Country

promoted to Partner in 2003. Prior to joining CHS, Mr. Lobo was employed by Nagelvoort
& Company, Inc., a private investment banking firm headquartered in New York. He holds
a B.S. from Georgetown University and an M.B.A. from Northwestern University. Mr.
Lobo serves on the Board of Directors of American Asphalt & Grading Company, AMF Bowling

Peter S. Pettit Vice President |
|
Mr. Pettit joined CHS in 2007 as a Vice President. Prior to joining CHS, he was employed
by CGW Southeast Partners, a private equity investment firm. He also worked for SunTrust
Robinson Humphrey, an investment bank headquartered in Atlanta. He holds a B.S. from the
University of Virginia and an M.B.A. from Northwestern University and serves on the Board of
Directors of Swank Audio Visuals LLC and TruckPro, Inc.

Cameron G. Smith Vice President |
|
Mr. Smith joined CHS in 2008 as a Vice President. Prior to joining CHS, he was employed by
TSG Capital Group, a private equity investment firm in Stamford, Connecticut. He also worked
for J.H. Whitney & Co., a private equity investment firm in Stamford, Connecticut.
Mr. Smith holds a B.S. from the Wharton School of the University of Pennsylvania as well as a
J.D. and an M.B.A. from Northwestern University. He is a licensed attorney in the State of Illinois
and a member of the Chicago and Illinois State Bar Associations. Mr. Smith serves on the Board of
Directors of Suture Express, Inc.

Mr. Code was a founder of Code Hennessy & Simmons in 1988. Prior to founding CHS, Mr. Code
was a Vice President with Citicorp's Leveraged Capital Group in Chicago. Before joining Citicorp,
he was employed by American National Bank in Chicago. He holds a B.A. and an M.B.A. from the
University of Iowa. Mr. Code serves on the Board of Directors of The Hillman Companies, Inc.,
KBA Holdings, Inc., MLS Holdings, Nautic Global Group, SCP Pool Corporation, HWF Holdings, Inc.,
Woodstream Corporation and American Laser Centers, LLC.
Mr. Code also serves on the Board of Directors of the University of Iowa Foundation, GAP Community
Center, and Resource Land Holdings and is currently chairing the Capital Campaign for Chicago's

Krista M. Hatcher Principal |

Ms. Hatcher joined CHS in 1999 and was promoted to Principal in 2007. Prior to joining
CHS, she was employed by the investment banking firm of J.P. Morgan & Company, Incorporated.
She holds a B.B.A. from the University of Michigan and an M.B.A. from the University of
Chicago. Ms. Hatcher serves on the Board of Directors of Nautic Global Group, Mainline
Supply Company and Woodstream Corporation.

Mr. Spinola was an Associate with CHS from 2003-2005 and has returned to CHS as a Vice President
in 2007. He was previously employed by Salomon Smith Barney in the Mergers and Acqusitions
Group of the Investment Banking Division. Mr. Spinola holds a B.B.A. from the University of
Notre Dame and an M.B.A. from Harvard Business School and serves on the Board of Directors of American Laser Centers, LLC.
He is also a C.F.A. Charterholder.
