Mr. Formolo joined CHS in 1990 and was promoted to Partner in 1997. Prior to joining CHS, Mr. Formolo was employed by Heller Financial, Inc. as a Vice President in the Leveraged Funding Group. He holds a B.B.A. from the University of Wisconsin and an M.B.A. from Northwestern University. Mr. Formolo serves on the Board of Directors of American Reprographics Company (NYSE: ARP); AMF Bowling Worldwide, Inc.; QubicaAMF Worldwide, S.a.r.L.; Heartland Dental Care, Inc.; KB Alloys, LLC; and Web Service Company, LLC. Mr. Formolo is also the Chairman of the Board of Trustees of North Shore Country Day School, is on the Dean's Advisory Board of the University of Wisconsin School of Business and serves on the Board of Directors of Chicago Fellowship and Swank Audio Visuals, LLC.

Mr. Smith joined CHS in 2008 as a Vice President. Prior to joining CHS, he was employed by TSG Capital Group, a private equity investment firm in Stamford, Connecticut. He also worked for J.H. Whitney & Co., a private equity investment firm in Stamford, Connecticut. Mr. Smith holds a B.S. from the Wharton School of the University of Pennsylvania as well as a J.D. and an M.B.A. from Northwestern University. He is a licensed attorney in the State of Illinois and a member of the Chicago and Illinois State Bar Associations.

Mr. Code was a founder of Code Hennessy & Simmons in 1988. Prior to founding CHS, Mr. Code was a Vice President with Citicorp's Leveraged Capital Group in Chicago. Before joining Citicorp, he was employed by American National Bank in Chicago. He holds a B.A. and an M.B.A. from the University of Iowa. Mr. Code serves on the Board of Directors of American Laser Centers, LLC; The Hillman Companies, Inc.; KBAlloys, LLC; Nautic Global Group; SCP Pool Corporation; and Woodstream Corporation.
