Mr. Formolo joined CHS in 1990 and was promoted to Partner in 1997. Prior to joining CHS, Mr. Formolo was employed by Heller Financial, Inc. as a Vice President in the Leveraged Funding Group. He holds a B.B.A. from the University of Wisconsin and an M.B.A. from Northwestern University. Mr. Formolo serves on the Board of Directors of American Reprographics Company, AMF Bowling Worldwide, Inc., KBA Holdings, Inc. and Suture Express, Inc. Mr. Formolo also serves on the Board of Trustees of North Shore Country Day School and the Board of Directors of Chicago Fellowship and G.A.P.C., Inc.

Mr. Lobo joined CHS in 1993 and after holding numerous positions at the firm, was promoted to Partner in 2003. Prior to joining CHS, Mr. Lobo was employed by Nagelvoort & Company, Inc., a private investment banking firm headquartered in New York. He holds a B.S. from Georgetown University and an M.B.A. from Northwestern University. Mr. Lobo serves on the Board of Directors of American Asphalt & Grading Company, AMF Bowling Worldwide, Inc., Qubica/AMF Products Worldwide, Swank Audio Visuals LLC, Inc. and Acton Mobile Industries.

Mr. Lhee joined CHS in 1997 and was promoted to Partner in 2007. Prior to joining CHS, he was employed by Morgan Stanley & Company in the Mergers and Acquisitions and Corporate Finance departments. He holds a B.S. and B.A. from the University of Pennsylvania and an M.B.A. from Northwestern University. Mr. Lhee serves on the Board of Directors of HWF Holdings LLC, Suture Express, Inc., Acton Mobile Industries and Swank Audio Visuals, LLC.

Mr. Pettit joined CHS in 2007 as a Vice President. Prior to joining CHS, he was employed by CGW Southeast Partners, a private equity investment firm. He also worked for SunTrust Robinson Humphrey, an investment bank headquartered in Atlanta. He holds a B.S. from the University of Virginia and an M.B.A. from Northwestern University and serves on the Board of Directors of Swank Audio Visuals LLC and TruckPro, Inc.

Mr. Smith joined CHS in 2008 as a Vice President. Prior to joining CHS, he was employed by TSG Capital Group, a private equity investment firm in Stamford, Connecticut. He also worked for J.H. Whitney & Co., a private equity investment firm in Stamford, Connecticut. Mr. Smith holds a B.S. from the Wharton School of the University of Pennsylvania as well as a J.D. and an M.B.A. from Northwestern University. He is a licensed attorney in the State of Illinois and a member of the Chicago and Illinois State Bar Associations. Mr. Smith serves on the Board of Directors of Suture Express, Inc.

Mr. Code was a founder of Code Hennessy & Simmons in 1988. Prior to founding CHS, Mr. Code was a Vice President with Citicorp's Leveraged Capital Group in Chicago. Before joining Citicorp, he was employed by American National Bank in Chicago. He holds a B.A. and an M.B.A. from the University of Iowa. Mr. Code serves on the Board of Directors of The Hillman Companies, Inc., KBA Holdings, Inc., MLS Holdings, Nautic Global Group, SCP Pool Corporation, HWF Holdings, Inc., Woodstream Corporation and American Laser Centers, LLC. Mr. Code also serves on the Board of Directors of the University of Iowa Foundation, GAP Community Center, and Resource Land Holdings and is currently chairing the Capital Campaign for Chicago's Inner City Young Life.

Mr. Brown joined CHS in 1994 and after holding numerous positions at the firm, was promoted to Partner in 2003. Prior to joining CHS, Mr. Brown was employed by Heller Financial, Inc. in Chicago, Illinois where he held various positions within Heller's Commercial Leveraged Lending and Real Estate Departments. He holds a B.B.A. from the University of Michigan and an M.B.A. from Northwestern University. Mr. Brown serves on the Board of Directors of Nautic Global Group, Waddington North America, Inc., and American Laser Centers LLC.

Ms. Hatcher joined CHS in 1999 and was promoted to Principal in 2007. Prior to joining CHS, she was employed by the investment banking firm of J.P. Morgan & Company, Incorporated. She holds a B.B.A. from the University of Michigan and an M.B.A. from the University of Chicago. Ms. Hatcher serves on the Board of Directors of Nautic Global Group, Mainline Supply Company and Woodstream Corporation.

Mr. Spinola was an Associate with CHS from 2003-2005 and has returned to CHS as a Vice President in 2007. He was previously employed by Salomon Smith Barney in the Mergers and Acqusitions Group of the Investment Banking Division. Mr. Spinola holds a B.B.A. from the University of Notre Dame and an M.B.A. from Harvard Business School and serves on the Board of Directors of American Laser Centers, LLC. He is also a C.F.A. Charterholder.

Mr. Hawkins joined CHS in 1995 and after holding numerous positions at the firm, was promoted to Partner in 2003. Prior to joining CHS, he was a Vice President with Heller Equity Capital Corporation where he was involved in organizing and managing private equity investments. He holds a B.S. from Indiana University and an M.B.A. from Northwestern University. Mr. Hawkins serves on the Board of Directors of American Asphalt & Grading Company, Kranson Holdings, Inc., Home Acres Building Supply Company, SunSource Holdings, Inc., TruckPro, Inc. and Mainline Supply Company.

Mr. Hogan joined CHS in 2000 and was promoted to Principal in 2007. Prior to joining CHS, he was employed by Lehman Brothers, Inc. in the Corporate Finance Department and prior to that, he was employed by the United States Central Intelligence Agency. He holds a B.A. from the University of Notre Dame and an M.B.A. from Northwestern University. Mr. Hogan serves on the Board of Directors of Kele, Inc., Home Acres Building Supply Co., Penhall LVI LLC, Houston Wire & Cable Company and SunSource Holdings, Inc.

Mr. Knoch joined CHS in 2000 and was promoted to Principal in 2007. Prior to joining CHS, he was employed by Mesirow Financial in the Corporate Finance Department and prior to that, he was employed by Arthur Andersen, LLP. He is a registered Certified Public Accountant and holds a B.S. from the University of Illinois and an M.B.A. from the University of Chicago. Mr. Knoch serves on the Board of Directors of Kranson Holdings, Inc., TruckPro, Inc., Penhall LVI LLC and AMF Bowling Worldwide, Inc.

Mr. Hennessy was a founder of Code Hennessy & Simmons in 1988. Prior to founding Fund I, Mr. Hennessy was a Vice President with Citicorp Mezzanine Investments and Citicorp's Leveraged Capital Group in Chicago. Before joining Citicorp, he was employed by Continental Bank. He holds a B.A. from Boston College and an M.B.A. from the University of Michigan. Mr. Hennessy serves on the Board of Directors of Gundle/SLT Environmental, Inc., Waddington North America, Inc. and Penhall LVI LLC. Mr. Hennessy also serves on the Board of Directors of the John G. Shedd Aquarium, and Children's Memorial Hospital and the Board of Trustees of the Greater Illinois Chapter of the National Multiple Sclerosis Society.

Mr. Simmons was a founder of Code Hennessy & Simmons in 1988. Prior to founding Fund I, Mr. Simmons was a Vice President with Citicorp's Leveraged Capital Group in Chicago. Before joining Citicorp, he was employed by Mellon Bank in Pittsburgh and Chicago. He holds an A.B. from Cornell University. Mr. Simmons serves on the Board of Directors of Gundle/SLT Environmental, Inc., MLS Holdings, Inc. and Penhall LVI LLC. Mr. Simmons also serves on the Board of Directors of The Latin School, Lincoln Park Zoo, the Greater Chicago Food Depository and the Visiting Committee of the University of Chicago School of Social Service.

Mr. George joined CHS in 1997 and was promoted to Partner in 2007. Prior to joining CHS, he was employed by Heller Financial, Inc. in the Corporate Finance Group. He also worked for KPMG. He holds a B.B.A. from the University of Notre Dame and an M.B.A. from the University of Chicago. Mr. Hendrie joined CHS in 2007 as a Vice President. Prior to joining CHS, he was employed by Goldman Sachs & Company in the Industrials Group of the Investment Banking Division in New York. Mr. Hendrie holds a B.S.E and M.B.A from the University of Michigan.

Mr. Asfour joined CHS as an Associate in 2007. Prior to joining CHS, he was employed by the investment bank of J.P. Morgan & Company in the Global Mergers and Acquisitions Group in New York. Mr. Asfour holds a B.S. from the Kelley School of Business at Indiana University.

Ms. Bonnema joined CHS as an Associate in 2007. Prior to joining CHS, she was employed by Wachovia Capital Markets LLC in the Mergers and Acquisitions Group of the Investment Banking Division. Ms. Bonnema holds a B.S. from Vanderbilt University.

Mr. Etmekjian joined CHS in 2007 as an Associate. Prior to joining CHS, he was employed by Robert W. Baird & Co. Inc. in the Consumer and Industrial Group of the Investment Banking Division. Mr. Etmekjian holds a B.A. from Northwestern University.

Ms. Jackson joined CHS as an Associate in 2006. Prior to joining CHS, she was employed by Credit Suisse First Boston in the Investment Banking Division. Ms. Jackson holds a B.S. from the University of Missouri.

Mr. Singh joined CHS as an Associate in 2007. Prior to joining CHS, he was employed by Citigroup Global Markets Inc. in the Investment Banking Division. Mr. Singh holds a B.A. from the University of Michigan.
