Mr. Formolo joined CHS in 1990 and was promoted to Partner in 1997. Prior to joining CHS, Mr. Formolo was employed by Heller Financial, Inc. as a Vice President in the Leveraged Funding Group. He holds a B.B.A. from the University of Wisconsin and an M.B.A. from Northwestern University. Mr. Formolo serves on the Board of Directors of American Reprographics Company (NYSE: ARP); AMF Bowling Worldwide, Inc.; QubicaAMF Worldwide, S.a.r.L.; Heartland Dental Care, Inc.; KB Alloys, LLC; and Web Service Company, LLC. Mr. Formolo is also the Chairman of the Board of Trustees of North Shore Country Day School, is on the Dean's Advisory Board of the University of Wisconsin School of Business and serves on the Board of Directors of Chicago Fellowship and Swank Audio Visuals, LLC.

Mr. Lobo joined CHS in 1993 and after holding numerous positions at the firm, was promoted to Partner in 2003. Prior to joining CHS, Mr. Lobo was employed by Nagelvoort & Company, Inc., a private investment banking firm headquartered in New York. He holds a B.S. from Georgetown University and an M.B.A. from Northwestern University. Mr. Lobo serves on the Board of Directors of Acton Mobile Industries; American Asphalt & Grading Company; AMF Bowling Worldwide, Inc.; QubicaAMF Worldwide, S.a.r.L.; Swank Audio Visuals, LLC; and Web Service Company, LLC.

Mr. Lhee joined CHS in 1997 and was promoted to Partner in 2007. Prior to joining CHS, he was employed by Morgan Stanley & Company in the Mergers and Acquisitions and Corporate Finance Departments. He holds a B.S. and B.A. from the University of Pennsylvania and an M.B.A. from Northwestern University. Mr. Lhee serves on the Board of Directors of Acton Mobile Industries; Heartland Dental Care, Inc.; and Swank Audio Visuals, LLC.

Mr. Smith joined CHS in 2008 as a Vice President. Prior to joining CHS, he was employed by TSG Capital Group, a private equity investment firm in Stamford, Connecticut. He also worked for J.H. Whitney & Co., a private equity investment firm in Stamford, Connecticut. Mr. Smith holds a B.S. from the Wharton School of the University of Pennsylvania as well as a J.D. and an M.B.A. from Northwestern University. He is a licensed attorney in the State of Illinois and a member of the Chicago and Illinois State Bar Associations.

Mr. Code was a founder of Code Hennessy & Simmons in 1988. Prior to founding CHS, Mr. Code was a Vice President with Citicorp's Leveraged Capital Group in Chicago. Before joining Citicorp, he was employed by American National Bank in Chicago. He holds a B.A. and an M.B.A. from the University of Iowa. Mr. Code serves on the Board of Directors of American Laser Centers, LLC; The Hillman Companies, Inc.; KBAlloys, LLC; Nautic Global Group; SCP Pool Corporation; and Woodstream Corporation. Mr. Code also serves on the Board of Directors of the University of Iowa Foundation and Resource Land Holdings.

Mr. Brown joined CHS in 1994 and after holding numerous positions at the firm, was promoted to Partner in 2003. Prior to joining CHS, Mr. Brown was employed by Heller Financial, Inc. in Chicago where he held various positions within Heller's Commercial Leveraged Lending and Real Estate Groups. He holds a B.B.A. from the University of Michigan and an M.B.A. from Northwestern University. Mr. Brown serves on the Board of Directors of American Laser Centers, LLC and Nautic Global Group. Mr. Brown also serves on the Board of Directors of the Illinois Policy Institute and Catalyst Schools.

Ms. Hatcher joined CHS in 1999 and was promoted to Principal in 2007. Prior to joining CHS, she was employed by the investment banking firm of J.P. Morgan & Company, Incorporated. She holds a B.B.A. from the University of Michigan and an M.B.A. from the University of Chicago. Ms. Hatcher serves on the Board of Directors of MSC Waterworks; Nautic Global Group; and Woodstream Corporation.

Mr. Spinola joined CHS as an Associate in 2003 and returned as a Vice President in 2007. He was previously employed by Salomon Smith Barney in the Mergers and Acqusitions Group of the Investment Banking Division. He holds a B.B.A. from the University of Notre Dame, an M.B.A. from Harvard Business School and is a C.F.A. Charterholder. Mr. Spinola serves on the Board of Directors of American Laser Centers, LLC.

Mr. Hawkins joined CHS in 1995 and after holding numerous positions at the firm, was promoted to Partner in 2003. Prior to joining CHS, he was a Vice President with Heller Equity Capital Corporation where he was involved in organizing and managing private equity investments. He holds a B.S. from Indiana University and an M.B.A. from Northwestern University. Mr. Hawkins serves on the Board of Directors of Home Acres Building Supply Company; Kele, Inc.; Kranson Industries, Inc.; MSC Waterworks; and SunSource Holdings, Inc.

Mr. Hogan joined CHS in 2000 and was promoted to Principal in 2007. Prior to joining CHS, he was employed by Lehman Brothers, Inc. in the Corporate Finance Department and prior to that, he was employed by the United States Central Intelligence Agency. He holds a B.A. from the University of Notre Dame and an M.B.A. from Northwestern University. Mr. Hogan serves on the Board of Directors of Kele, Inc.; LVI Services Inc.; and SunSource Holdings, Inc.

Mr. Knoch joined CHS in 2000 and was promoted to Principal in 2007. Prior to joining CHS, he was employed by Mesirow Financial in the Corporate Finance Department and prior to that, he was employed by Arthur Andersen, LLP. He is a registered Certified Public Accountant, holds a B.S. from the University of Illinois and an M.B.A. from the University of Chicago. Mr. Knoch serves on the Board of Directors of AMF Bowling Worldwide, Inc.; Kranson Industries, Inc.; and Penhall LVI LLC.

Mr. Brown joined CHS as an Associate in 2003 and returned as a Vice President in 2008. Prior to joining CHS , Mr. Brown was employed by Salomon Smith Barney in the Investment Banking Division. He holds a B.B.A. from the University of Notre Dame, an M.B.A. from Harvard Business School and is a C.F.A. Charterholder. Mr. Brown serves on the Board of Directors of MSC Waterworks.

Mr. Hennessy was a Founder of Code Hennessy & Simmons in 1988. Prior to founding Fund I, Mr. Hennessy was a Vice President with Citicorp Mezzanine Investments and Citicorp Leveraged Capital Group in Chicago. Before joining Citicorp, he was employed by Continental Illinois National Bank. He holds a B.A. from Boston College and an M.B.A. from the University of Michigan. Mr. Hennessy serves on the Board of Directors of Gundle/SLT Environmental, Inc.; LVI Services Inc.; Penhall International Corporation; and Thermon Group Holdings, Inc. Mr. Hennessy also serves as a Trustee of the John G. Shedd Aquarium, The Museum of Science and Industry and the Greater Illinois Chapter of the National Multiple Sclerosis Society and is a Director of the Children's Memorial Hospital and Foundation.

Mr. Simmons was a founder of Code Hennessy & Simmons in 1988. Prior to founding Fund I, Mr. Simmons was a Vice President with Citicorp's Leveraged Capital Group in Chicago. Before joining Citicorp, he was employed by Mellon Bank in Pittsburgh and Chicago. He holds an A.B. from Cornell University. Mr. Simmons serves on the Board of Directors of Gundle/SLT Environmental, Inc.; LVI Services Inc.; and Thermon Group Holdings, Inc. Mr. Simmons also serves on the Board of Directors of The Latin School, Lincoln Park Zoo, the Greater Chicago Food Depository and the Visiting Committee of the University of Chicago School of Social Service.

Mr. George joined CHS in 1997 and was promoted to Partner in 2007. Prior to joining CHS, he was employed by Heller Financial, Inc. in the Corporate Finance Group. He also worked for KPMG. He holds a B.B.A. from the University of Notre Dame and an M.B.A. from the University of Chicago. Ms. Lester joined CHS as an Associate in 2004 and returned as a Vice President in 2008. Prior to joining CHS, she was employed by Lehman Brothers, Inc. in the Investment Banking Division. Ms. Lester holds a B.A. from Cornell University and an M.B.A. from Northwestern University.

Mr. Bronson joined CHS as an Associate in 2010. Prior to joining CHS, he was employed by William Blair & Company in the Investment Banking Division. He holds a B.A. from Northwestern University.

Mr. Kunz joined CHS as an Associate in 2009. Prior to joining CHS, he was employed by Citigroup Global Markets, Inc. in the Global Investment Banking Division. He holds a B.S. and an M.S. from the University of Illinois at Urbana-Champaign.

Mr. Mellors joined CHS as an Associate in 2009. Prior to joining CHS, he was employed by William Blair & Company, LLC in the Investment Banking Division and prior to that, he was employed by Gilbane Building Company as an Engineer. He holds a B.S. from Brown University.

Mr. Trzebiatowski joined CHS as an Associate in 2010. Prior to joining CHS, he was employed by RC2 Corp. in the Corporate Development and Business Strategy Group, and prior to that, he was employed by Robert W. Baird in the Investment Banking Division. He holds a B.B.A. from the University of Wisconsin.
